State Health Plan/Change Forms

If you are a member of the State Health Plan, your health insurance premium payments are deducted on a pre-tax basis. The Flexible Benefit Plan (IRS Section 125) allows any premiums you pay for health benefit coverage to be deducted from your paycheck before taxes are withheld. If you elect to have your premiums paid on a before tax basis, your health benefit coverage can only be changed (dependents added or dropped) during the annual enrollment period (effective July 1st)or when a qualifying event occurs. When one of these events occur you must complete a Change Form and forward it to the Human Resources Office by the 16th of the PRIOR month if you want your changes to take effect the following month. EXAMPLE: "If you know you need to change your Health coverage effective December 1st, HR needs your Change Form by NOVEMBER 16th. Premiums will be deducted from your paycheck in the month of November for December coverage. Must report the event causing the change within 30 days.
