Due to the benefit change and rate increases, the State Health Plan will conduct a Fall Enrollment for 2007-2009 to allow employees to switch plans. Completed forms are due in the Human Resouces Office no later than August 31, 2007. Enrollment changes will become effective October 1st, 2007. Prescription Drug Co-payment for Preferred brand with no generic equivalent for a 34-day supply incrased to $30. (Eff. 8/6/07). Indemnity Plan deductible increase to $450 & Fmly deductible to $1,350. Office visits co-pay increase to $25. Under the PPO Plans, Therapy visits is now unlimited. Chiropractic Visits increase to 30. Increased Mental Health & Chemical Dependency Visits. Rate changes are effective OCTOBER 1ST, 2007.
