The NCFlex Annual Enrollment for 2008 will be from October 1 through November 2, 2007. The following is an overview of the changes for the up and coming annual enrollment: DENTAL…United Concordia will replace the current carrier with a slight rate change. Significant changes in plan benefits, *Low option annual deductible decrease to $25/75 (individual/family), from $50/150.*High option calendar year maximum increased to $1,250 from $1,000. *Exams, cleaning, fluoride treatments and x-ray available twice per year (instead of once every 6 months) *Replacement fillings available once every 12 months (instead of 24 months) as needed. *Periodontal surgery covered once in a 24-month period (instead of a 36-month period.*Removed $750 calendar year maximum for orthodontics; $1,500 lifetime maximum remains the same. *Ongoing orthodontics reimbursements will be automatically paid once the initial treatment plan is received. *Certain waiting periods will be waived for employees enrolled in an after-tax dental plan for any length of time FLEXIBLE SPENDING ACCOUNTS Monthly contribution limits removed; new hires or participants experiencing a qualified life event may elect the full calendar year maximum regardless of the number of months left in the year. GROUP TERM LIFE If not currently enrolled, for 2008, purchase $20,000 of coverage on a guaranteed issue basis (unless previously denied). If currently enrolled, add $10,000 of additional coverage each annual enrollment up to the guaranteed issue amount. SUPPLEMENTAL MEDICAL Coverage will no longer be offered beginning January 1, 2008.
