If your address has changed, please submit the completed forms to the Human Resources Office within 30 days:
1. Payroll Name/Address Change Form.
2. Retirement System Address Change Form.
3. State Health Plan Change Form.
If you are participating in the following Plans, you may also complete the following change of address forms:
1. NCFlex Family/Employment Status Change Form
2. Fortis Dental Plan Form
3. And any other Miscellaneous deductions you may have.
